Six Nations Family Health Team

The Six Nations Family Health Team located in Ohsweken is a fitness centre that focuses on fostering health and wellness within the community. The centre is part of the White Pines Wellness Centre and is staffed by a diverse multidisciplinary team of practitioners of Haudenosaunee and Western medicine. The primary mission of the team is to build and maintain a healthy community through patient-centered wholistic care in partnership with surrounding resources.

The facility offers a range of services, including clinic visits, access to visiting professionals, and intake/referral services. The centre also houses the Internal Medicine Rapid Assessment Clinic (IMRAC) in collaboration with St. Joseph's Hospital of Hamilton, providing rapid internal medicine assessments locally for Indigenous patients. Patient care is delivered in a culturally safe manner, with the option for patients to incorporate traditional healing practices and allied health services into their care plan.
