Care Assess – South, located in Rosny Park, is a leading provider of aged care services, community health assessments, disability services and support, health consulting, nursing agency placements, occupational therapy, physical fitness programmes, social work, and wellness programmes in the region. Since 1999, Care Assess has been dedicated to providing independent and unbiased assessments to individuals and their families, separate from direct service provision.

As a proud Tasmanian organisation, Care Assess recognises and pays sincere respect to the Tasmanian Aboriginal Community as the original owners and custodians of the land on which they operate. They honour and acknowledge the Elders, both past and present.

Care Assess offers an array of services tailored to meet the needs of their diverse clientele. They provide Regional Assessment Services on behalf of My Aged Care, conducting assessments for aged care clients across Tasmania under the contract of the Department of Health on behalf of the Australian Government.

In addition to aged care assessments, Care Assess also fulfils the role of the Veterans' Home Care Assessment Agency, coordinating and assessing home care services for veterans throughout Tasmania. They are contracted by the Department of Veterans Affairs on behalf of the Australian Government.

Furthermore, Care Assess has been selected by the Department of Health to participate in a residential aged care funding reform trial across Tasmania, conducting residential care funding assessments as part of this initiative.

Care Assess also plays a significant role in providing comprehensive health assessments and care plans on behalf of General Practises. They work closely with GPs to manage chronic diseases under Medicare's chronic diseases management items.

One of the latest developments by Care Assess is their participation in the Department of Health's new Integrated Assessment Tool Live Trial (IAT trial). This trial aims to enhance the assessment process through the use of an integrated tool for better outcomes and improved services. Care Assess is thrilled to be part of this trial, which commenced in April 2023 and will run until June 2023.
